Care you needed and did not get leaves no row in any federal cost file: MEPS’s event files, HCUP, Medicare claims. National surveys (NHIS, the National Health Interview Survey, and MEPS’s own access-to-care questions) do count that it happened, to a share of the population; none of them attaches a dollar to it, and none of them is yours. This is the count of it, one report at a time.

Was there care you knew you needed but did not go and get?

Why no dataset has this

A visit that never happened generates no claim, no encounter record and no expenditure event. NHIS and MEPS's own access-to-care questions do record that care was delayed or not received because of cost, for a share of the population, but no federal cost file attaches a dollar to it, and none of them is about you.

Care you asked for and were refused

Did a clinician or insurer refuse something you asked for?

Why no dataset has this

A denial is recorded by the payer, not in expenditure data, and a clinician declining to order a test leaves no trace at all. The need existed and is uncounted.

Times you were told it was nothing

How many times were you told your symptoms were anxiety, stress, or nothing at all?

  • “told to lose weight”
  • “told it was stress”
Why no dataset has this

The visit is billed, so it appears in the data as care delivered. What the data cannot show is that it produced no answer. A dismissal and a diagnosis cost the same and look the same in an expenditure file.

Care aimed at the wrong thing

Were you treated for something you did not have?

  • “treated for depression instead”
  • “wrong diagnosis for two years”
  • “medication for the wrong condition”
Why no dataset has this

This care is fully recorded and fully paid, and it is counted as successful utilization. Nothing in the data marks it as care that was aimed at the wrong target.

Time spent searching rather than being treated

Roughly how long from your first symptom to an answer?

Why no dataset has this

Federal expenditure files have no field for duration of an undiagnosed period. The clock only starts when a code is assigned, so the longest and most expensive part of an odyssey begins after the data starts counting.

What the search cost you outside of medicine

What did this take from you that no bill records?

Why no dataset has this

No health dataset has a field for this. Job loss, school, a marriage: no federal file has ever counted them, so the only way they exist as a number is if the people who carried them say so.

Why counting it is the whole point

A visit that never happened, a test that was refused and a month spent waiting leave nothing behind in MEPS’s event files, in HCUP or in a claims file.
